Under Florida Statute Section 817.234, Jacksonville Insurance Fraud occurs when a person a person has an intent to injure or defraud an insurer and:

1. presents a statement to the insurer for an insurance claim knowing that the statement is false, incomplete, or misleading about a material fact;
2. prepares a statement intended to be presented to an insurer in connection with an insurance claim knowing that the statement is false, incomplete, or misleading about a material fact;
   
3. presents or prepares a false statement that is a material fact, knowing it will be used in the application of an insurance policy; or
   
4. conceals a material fact in the application process.
